This calculation helps determine the correct survivor benefit amount when a worker dies before reaching age 62.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In some cases, Social Security employees manually processed survivor claims but failed to properly apply the WINDEX calculation. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As a result, the Primary Insurance Amount, or PIA, was calculated incorrectly. Since the PIA is used to determine monthly benefits, the mistake caused some widows and widowers to receive smaller checks than they should have received.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Social Security Administration said it began taking action after receiving the audit report. The agency reportedly started reminding field office employees about the correct procedures for calculating widow and widower benefits.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Who Was Affected by the Social Security Error?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The payment mistake mainly affected widows and widowers whose spouses died before age 62. Survivors whose claims required manual processing were also more likely to be impacted, because manual calculations created a greater chance of human error.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The watchdog report suggested that a significant share of surviving spouses could have been affected during the review period. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These errors are especially concerning because many widows and widowers rely on survivor benefits as a financial lifeline after losing a spouse.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For seniors living on fixed incomes, underpayments can make it harder to cover rent, groceries, utilities, medical bills and other basic expenses.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Another Problem: Unclear Filing Guidance<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The report also pointed to a separate issue involving unclear Social Security guidance. Auditors estimated that 5,367 widows and widowers may have missed out on about $114 million in additional benefits because they were not properly informed about their filing options.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In some cases, beneficiaries may have been better off collecting survivor benefits first while delaying retirement benefits. This strategy can sometimes allow a person\u2019s own retirement benefit to grow before they claim it later.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">However, if beneficiaries were not clearly told about this option, they may have claimed early and locked themselves into permanently lower payments. The report estimated that these missed opportunities cost affected individuals an average of more than $21,000 each.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Is Social Security Doing Now?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The Inspector General recommended that the SSA identify affected widows and widowers, correct benefit calculations and issue retroactive payments where underpayments occurred.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The report also urged the agency to improve employee training and make sure beneficiaries receive complete information about their filing choices. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The SSA said it is reviewing the cases identified in the audit and will take corrective action when needed.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Should Widows and Widowers Do?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Surviving spouses who believe they may have been underpaid should contact the Social Security Administration and request a review of their survivor benefit calculation.
